Patient Identification
All patients are required to present a valid government-issued photo ID at the time of their appointment. This policy is in place to ensure patient safety, accurate medical records, and compliance with medical and regulatory standards.
Appointments may not be conducted without proper identification.
Photo Documentation
For medical, aesthetic, and treatment-planning purposes, photographs are taken prior to treatment. These images are used to document baseline conditions, track progress, and support clinical decision-making.
All photos are maintained in accordance with HIPAA and patient privacy standards and are kept strictly confidential.
Consent Forms
All patients are required to review and sign applicable consent forms prior to receiving any treatment at BODYWELLE. Consent forms ensure patients understand the nature of the treatment, potential risks, benefits, and alternatives.
Patients who wish to complete consent forms ahead of time at home may request them in advance. If consent forms are not requested beforehand, they will be completed in-office at the time of the appointment.
