Enclomiphene vs. Anastrozole for Low Testosterone: How They Work and Who May Benefit

Licensed physician and Founder & Medical Director of BODYWELLE. Dr. Martin earned his MD from Michigan State University and trained in Family Medicine during a residency at the University of California, Irvine.

Low testosterone is often treated by adding testosterone from the outside, through injections, gels, or pellets. That approach works well for many men, but it comes with a tradeoff that matters to anyone who wants to preserve fertility. Enclomiphene and anastrozole take a different route. Rather than replacing testosterone, these oral medications signal the body to produce more of its own.

Dr. Alonso Martin, founder of BODYWELLE in Miami Beach, evaluates men’s hormone health within a broader longevity framework. His background in neuroscience and family medicine informs how he approaches the brain-to-testicle signaling pathway that governs testosterone production, and how he decides when a replacement strategy or a stimulation strategy is the better fit. Both enclomiphene and anastrozole are prescribed off-label for this purpose, meaning raising testosterone is not an FDA-approved use, but specialists prescribe them based on clinical experience and research.

Why Not Just Take Testosterone?

Standard testosterone replacement therapy delivers testosterone from outside the body. It reliably relieves symptoms such as fatigue, low libido, and reduced muscle mass. The problem is what it does to the body’s own signaling.

When the brain senses high testosterone levels coming in from an external source, it stops sending its own signals to the testicles. Over time, the testicles can shrink and sperm production can fall. For a man who hopes to father children, that is a significant cost.

Enclomiphene and anastrozole avoid this shutdown. They work upstream, prompting the brain and testicles to keep doing their jobs. Testosterone rises while sperm production continues.

How Enclomiphene Works

Enclomiphene belongs to a class of medications called selective estrogen receptor modulators, or SERMs. A SERM blocks estrogen’s effect in specific tissues while leaving it active in others.

In the brain, estrogen acts as a brake on testosterone production. When estrogen binds to receptors in the hypothalamus and pituitary gland, it tells them to slow the release of two hormones, luteinizing hormone (LH) and follicle-stimulating hormone (FSH). Enclomiphene blocks that brake. The brain responds by sending more LH and FSH to the testicles, which then produce more testosterone and continue producing sperm.

Enclomiphene is taken as a pill. In studies, it raised testosterone into the normal range at levels comparable to testosterone gel, while maintaining or improving sperm counts.

Who May Benefit From Enclomiphene

Enclomiphene is best suited to men whose low testosterone originates in the signaling pathway rather than in the testicles themselves. This is sometimes called secondary hypogonadism. The testicles are capable of producing testosterone; they are simply not receiving a strong enough signal to do so.

It is a particularly good fit for men who want to maintain fertility while addressing symptoms of low testosterone. Because it keeps LH and FSH flowing, sperm production is supported rather than suppressed.

How Anastrozole Works

Anastrozole is an aromatase inhibitor. Aromatase is the enzyme that converts testosterone into estrogen. Every man carries some aromatase, and some of his testosterone is always being converted. Anastrozole slows that conversion.

Lower estrogen has two effects. First, it eases the brake on the brain, so LH and FSH release increases and the testicles produce more testosterone. Second, it shifts the testosterone-to-estrogen ratio in a more favorable direction, which matters for men whose symptoms are driven as much by excess estrogen as by low testosterone.

Anastrozole is also taken as a pill. It typically raises testosterone by roughly 50 percent, usually into the normal range. In head-to-head studies, it tended to raise testosterone somewhat less than SERMs but produced a more favorable testosterone-to-estrogen ratio.

Who May Benefit From Anastrozole

Anastrozole makes the most sense for men with low testosterone and a relatively high estrogen level. This pattern is common in men carrying extra weight, because fat tissue contains aromatase and converts more testosterone into estrogen. In that setting, lowering estrogen addresses the mechanism behind the imbalance.
Like enclomiphene, anastrozole does not suppress sperm production, so it remains an option for men who want to preserve fertility.

Choosing Between Them

The right choice depends on what the bloodwork shows. A man with low testosterone, low or low-normal LH and FSH, and an estrogen level within range is a candidate for enclomiphene. A man with low testosterone and an elevated estrogen level, particularly if he carries excess body fat, may respond better to anastrozole.

Some men fall somewhere in between, and the decision rests on the full clinical picture rather than any single number. Symptoms, body composition, fertility goals, and other health factors all inform the choice. This is why hormone evaluation at BODYWELLE begins with a comprehensive panel rather than a single testosterone reading.

Follow-up bloodwork helps track testosterone, estrogen, LH, and FSH during treatment.

What Monitoring Looks Like

Both medications require follow-up blood tests to fine-tune the dose and confirm the medication is working. Testosterone and estrogen are measured, and LH and FSH are often checked as well.

The goal is to bring testosterone into the normal range without pushing estrogen too low. Estrogen plays a role in bone density, cardiovascular health, and libido in men, so eliminating it is not the aim. Dosing is adjusted based on how the numbers and the symptoms respond together.

Monitoring also confirms that the pathway is responding as expected. If testosterone does not rise with enclomiphene, for example, the problem may lie in the testicles rather than the brain, and a different approach may be warranted.

Where Hormone Health Fits Into Men’s Wellness

Hormone health is evaluated alongside energy, body composition, sexual function, and overall wellness.

Testosterone affects energy, mood, body composition, sexual function, and long-term health. Low levels can contribute to fatigue, reduced muscle, increased body fat, and diminished libido. Addressing the hormone picture is often a foundation for other concerns.

Men’s sexual wellness care at BODYWELLE considers hormones alongside circulation, desire, and overall health. When testing supports it, hormone-based treatment may be recommended as part of a broader plan. The team coordinates testing, follow-up, and adjustments so the plan stays aligned with each patient’s goals.
